Pengaruh perubahan sop perawatan luka pasca sectio caesarea terhadap infeksi luka operasi di RSUD Karimun

2008 | Tesis | S2 Ilmu Kesehatan MasyarakatLatar belakang: Medical error sering dihubungkan dengan infeksi nosokomial, salah satunya adalah Infeksi Luka Operasi (ILO). Infeksi tersebut menyerang pasien yang menjalani operasi/tindakan pembedahan baik di bagian bedah maupun di bagian lain, termasuk bagian Kebidanan dan Kandungan. Infeksi sesudah operasi sectio caesarea dapat mengakibatkan peningkatan angka morbiditas dan mortalitas ibu. Selama tahun 2007 dari 295 tindakan sectio caesarea didapatkan ILO 24 kasus di RSUD Karimun. Apabila dikaitkan dengan factor penyebab ILO, adanya kasus ILO di RSUD Karimun diduga akibat pekerjaan dari perawat di ruang rawat yang tidak sesuai dengan evidence based dari Standard Operating Procedures (SOP) perawatan luka yang ada. Tujuan penelitian: Mengetahui dan mengidentifikasi kasus ILO, mengetahui dampak intervensi perubahan SOP perawatan luka operasi terhadap angka kejadian ILO dan mencari faktor-faktor risiko ILO. Metode: Jenis penelitian yang digunakan adalah Quasi Experimental menggunakan rancangan the Separate-Sample pretest and postest tanpa grup kontrol. Populasi penelitan adalah semua pasien yang melakukan operasi sectio caesarea yang dirawat di ruang rawat Kebidanan dan Kandungan dan pasien yang sudah pulang dari rumah sakit. Variabel dependen pada penelitian mini adalah angka kejadian ILO sedangkan variabel independennya berupa perawatan luka. Instrumen pengumpulan data menggunakan check list untuk pretest dan postest. Pengolahan data menggunakan uji analisi independent t test dan uji statistik chi square. Hasil penelitian: Perubahan Standard Operating Procedures perawatan luka di ruang rawat Kebidanan dan Kandungan terbukti secara bermakna menurunkan angka kejadian ILO (p< 0,05). Kesimpulan: Intervensi yang dilakukan terhadap perawatan luka dapat menurunkan angka kejadian ILO. Faktor risiko ILO adalah kelompok subyek dengan umur berisiko tinggi persalinan
Background. Medical error is commonly correlated with nosocomial infection, among others is Post Operative Wound Infection. This infection can be suffered by post op patients, whether in surgical department or others such as the gyneacology department. Infection occuring post a caesarean operation can caused an increasing rate of maternal morbidity and mortality. During 2007, from 295 performed caesarean sections, there were 24 reported cases of post operative wound infection in Karimun General Hospital. In the relation of causing factors of post operative wound infection, cases found in Karimun General Hospital are suspected due to the incapability of nurses in dressing rooms that were not appropriate with the current evidence-based standard operational procedures. Objectives. This study was performed to identify and explain post operative wound infection cases, to analyze the effect of the intervention in standard operational procedures towards the incidence of post operative wound infection cases and to seek risk factors for this disease. Methods. This was an experimental study using pre and post test study design without control group. Study population was all staff in gyneacology ward, while study subject were caesarean section inpatients and discharged patients. Dependent variable of this study is incidence of Post Operative Wound Infection, while independent variable is wound treatment. Instruments used to gather data were check lists for pre and post test. Data analysis was performed using independent t test and chi square test. Results. The changes of standard operational procedure for wound bandaging in gyneacology ward were significantly decreasing the incidence of Post Operative Wound Infection cases (p<0.05). Conclusion. Intervention that was done to wound bandaging and treatment procedures can decrease the incidence of Post Operative Wound Infectetion cases. Common risk factor for Post Operative Wound Infection is subjects with high risk pregnancy due to maternal age.
