Evaluasi pengobatan massal diethylcabamazine dan albendazole pada filariasis bancrofti di Kabupaten Subang Propinsi Jawa Barat

2008 | Tesis | S2 Ilmu Kedokteran TropisPenelitian ini bertujuan untuk mengetahui persentase penurunan microfilaria rate, persentase penurunan kepadatan microfilaria dan persentase penurunan kepadatan rata-rata microfilaria setelah pengobatan kombinasi DEC dan albendazole di daerah endemis kabupaten Subang propinsi Jawa Barat serta mengetahui bermacam-macam efek samping pasca pengobatan kombinasi DEC & albendazole di kabupaten Subang Jawa Barat. Penelitian ini jenis penelitian studi observasional analitik dengan menggunakan desain before dan after trial. Data awal infeksi filariasis bancrofti dengan microfilaria positif dan microfilaria rate sebelum pengobatan DEC & albendazole diperoleh dari data sekunder DinKes Jawa Barat & DinKes Subang. Sesudah pengobatan DEC & albendazole cara pengumpulan data yaitu dengan kegiatan survei darah jari (SDJ) pada malam hari pukul 21.00 s/d 01.00. Jumlah sampel yang diambil SDJ adalah 130 orang di Kabupaten Subang Jawa Barat. Wawancara efek samping obat dengan menggunakan kuesioner pada subjek penelitian di 17 desa kabupaten Subang. Analisis data menggunakan uji statistik Paired t Test. Pengobatan kombinasi DEC & albendazole menurunkan microfilaria rate mencapai 100%, menurunkan kepadatan microfilaria/ml darah serta menurunkan kepadatan rata-rata microfilaria/ ml darah mencapai 100% di daerah endemis Kabupaten Subang propinsi Jawa Barat. Pengobatan kombinasi dengan diethylcarbamazine dan albendazole menimbulkan efek samping yang bermacam-macam pada 89.913 responden yang minum obat di daerah endemis filariasis bancrofti di Kabupaten Subang propinsi Jawa Barat. Efek samping yang ditimbulkan bervariasi antara lain berupa sakit kepala 8,34%, pusing 59,21%, demam 3,09%, mual 14,51%, muntah 4,50%, sakit sendi 0,83%, lesu 5,17%, gatal-gatal 2,17%, keluar cacing dari usus 0,33%, tidak ada yang mengeluh efek samping asma, wheezing dan diare. Total efek samping yang timbul pada pengobatan tahun ketiga adalah 1,33%.
Objective: The aims of this research was to identify evidence of the percentage reduction microfilaria rate, percentage reduction pattern of change parasite density and percentage reduction mean of parasite density after Mass Drug Administration with DEC and albendazole at District of Subang, Province of West Java. Knowing adverse reaction of Mass Drug Administration (MDA) with DEC and albendazole at District of Subang, Province of West Java. Method: The research was observation analytic study, design before and after trial. Collecting second data filariasis bancrofti with microfilaria positive, microfilaria rate before treatment with DEC & albendazole at District of Subang from Health Government Province of West Java and Health Government District of Subang Three years after MDA DEC & albendazole the data collection by peripheral blood finger at night 09.00 pm until 00.30. Sample was 130 people from District of Subang. Adverse reaction of therapy DEC & albendazole used questionnaire to give any interviews to responden at 17 village in District of Subang. These data were analyzed by using Paired t Test. Result: Results of microfilaria rate, mean of parasity density at Rancahilir & Curugrendeng village in District of Subang Province of West Java was 100% reduction. Mass Drug Administration DEC & albendazole cause many variation adverse reaction. The adverse reaction after MDA with DEC and albendazole were headache 8,34%, dizziness 59,21%, febris 3,09%, nausea 14,5%, vomit 4,5%, pain of joint 0,83%, fatique 5,17%, itching 7,17%, excretion of worm from the tractus intestina 0,33%, this data collecting from 89.913 responden whom drinking drug DEC & albendazole at endemic area filariasis bancrofti District of Subang. The total adverse reaction after the third year MDA is 1,33%.
