Peran Variasi Genetik Gen Nicotinamide Phosporybosyl Transferase (NAMPT) pada Penderita Sindroma Metabolik Etnis Jawa di Yogyakarta

Latar belakang: Prevalensi sindroma metabolik (MetS) terus meningkat di Indonesia, hal tersebut terkait peningkatan risiko mortalitas dan morbiditas akibat penyakit kardiovaskuler. Variasi gen NAMPT rs61330082 dan rs4730153 merupakan salah satu faktor yang mempengaruhi abnormalitas metabolisme lemak, inflamasi hingga kejadian cardiovaskuler dengan hasil yang berbeda pada berbagai populasi. Studi assosiasi fenotipe genotipe ini belum pernah dipublikasikan dengan populasi etnis Jawa sebelumnya. Tujuan: Penelitian ingin mengkaji peran variasi genetik gen NAMPT rs61330082, rs4730153 dan haplotipe keduanya terhadap MetS, komponen diagnosis MetS dan risiko atherosklerosis Metode: Studi ini merupakan studi kasus kontrol, 84 penderita MetS dan 84 kontrol etnis Jawa, matching sesuai usia dan jenis kelamin. Diagnosis MetS berdasarkan NCEP ATP-III modifikasi untuk populasi Asia, memenuhi minimal 3 dari kriteria obesitas sentral, hipertensi, hiperglikemia, hipertrigliseridemia dan LowHDL. Penilaian risiko atherosklerosis berdasarkan risiko atherogenik dari skor Atherogenic Index of Plasma (AIP). Genotiping dilakukan dengan teknik PCR-RFLP. Analisis bivariat maupun multivariat dilakukan untuk menilai peran genotipe. Hasil: Genotipe GG dari rs4730153 merupakan genotipe risiko terhadap MetS (p=0,038; OR 16,99; 95% CI 1,18-245,3) dibandingkan AA; GG (p=0,049; OR 2,92; 95% CI 1,01-8,48) genotipe risiko hipertensi dibandingkan AA/AG pada usia 45 tahun. Sebaliknya pada populasi > 45 tahun genotipe AG (p=0,026; OR 0,16; 95% CI 0,03-0,81) dan GG (p=0,19; OR 0,35; 95% CI 0,08-1,65) merupakan genotipe proteksi dibandingkan genotipe AA terhadap kejadian LowHDL. Genotipe CC dari rs61330082 merupakan genotipe proteksi dibandingkan TC/TT (p=0,015; OR 4,71; 95% CI 1,35-16,35) terhadap kejadian LowHDL pada usia 45 tahun. Peran variasi genetik NAMPT lebih tampak pada risiko atherosklerosis. Tanpa stratifikasi usia genotipe GG (p=0,04; OR 0,33; 95% CI 0,11-0,95) dan AG (p=0,049; OR 0,31; 95% CI 0,1-0,99) dari rs4730153 merupakan genotipe proteksi risko atherosklerosis sedang hingga berat dibandingkan genotipe AA. Hal serupa ditemukan pada usia > 45 tahun. Genotipe GG (p=0,17; OR 0,28; 95% CI 0,04-1,75) dan AG (p=0,013; OR 0,09; 95% CI 0,01- 0,61) merupakan genotipe proteksi dibandingkan genotipe AA; genotipe TT (p=0,029; OR 0,07; 95% CI 0,01-0,76) merupakan genotipe proteksi dibandingkan genotipe CC. AGTC merupakan haplogenotipe proteksi terhadap risiko atherosklerosis sedang hingga berat pada usia yang lebih tua Kesimpulan: Variasi genetik rs4730153 berperan pada Met-S untuk usia yang lebih muda. NAMPT rs61330082 dan rs4730153 berperan pada komponen diagnosis Met-S berupa hipertensi dan LowHDL serta risiko atherosklerosis. Genotipe risiko pada usia muda berbeda.

Introduction: Metabolic syndrome (MetS) prevalence increased in Indonesia, its associated with increasing mortality and morbidity related cardiovascular diseases. Interaction between genetic variants and environmental factor caused MetS phenotype. Genetic variant of NAMPT rs61330082 and rs4730153 associated to lipid metabolism abnormality, inflammation and cardiovascular diseases. The results of study varied in various population. This genotype-phenotype associated study never been published before with Javanese as participants. Objective of this study was to investigated association of the two genetic variants and its haplotype with MetS, MetS diagnose component and atherosclerosis risk. Methods: Study design was case and control, 168 subject Javanese who reside in Yogyakarta participated in this study. Case and control match for age and gender. MetS diagnose based on NCEP ATP-III modification for Asia population. Subject who have at least 3 criteria of central obesity, hypertension, hyperglycemia, hyper-triglycerides and low-HDL grouped as case of MetS. Atherosclerosis risk based on atherogenic zone of Atherogenic Index of Plasma (AIP). Genotyping perform with PCR-RFLP. Bivariate and multivariate analyzed associated between genotype, phenotype and its modifier. Logistic regression model used for interpreted this association Result: GG genotype of rs4730153 was risk genotype of MetS (p=0,038; OR 16,99; 95% CI 1,18-245,3) than AA; GG (p=0,049; OR 2,92; 95% CI 1,01- 8,48) also risk genotype for hypertension than AA/AG for age 45 years old. Contrary with it AG (p value 0,038; OR 0,19; 95%CI 0,04-0,92) and GG (p value 0,186; OR 0,35; 95%CI 0,08-1,65) was protection genotype than AA genotype for low-HDL in overall population. CC genotype rs61330082 was protection than TC/TT (p=0,015; OR 4,71; 95% CI 1,35- 16,35) for low-HDL at age 45 years old. Association of this genetic variant more prominent in atherosclerosis risk. Overall population GG (p=0,04; OR 0,33; 95% CI 0,11-0,95) and AG (p=0,049; OR 0,31; 95% CI 0,1-0,99) was protective genotype for moderate to high atherosclerosis risk than AA. For older age group (>45 years old) GG genotype (p=0,04; OR 0,33; 95% CI 0,11-0,95) and AG genotype (p=0,049; OR 0,31; 95% CI 0,1-0,99) was protection than AA genotype and TT (p=0,029; OR 0,07; 95% CI 0,01-0,76) genotype was protection than CC genotype. Conclusion rs4730153 associated with MetS and hypertension in younger age, both genetic variant associated with low-HDL and atherosclerosis risk. Younger age has difference genotype risk.
