Infeksi Sitomegalovirus Kongenital

2016 | Tesis-Spesialis | SP ILMU KESEHATAN ANAKInfeksi sitomegalovirus (CMV) merupakan salah satu masalah kesehatan yang banyak dijumpai di negara-negara berkembang, termasuk Indonesia. Sebuah penelitian terdahulu melaporkan bahwa tanpa pemberian ganciclovir intravena, infeksi CMV yang bersifat kongenital pada seorang bayi berakibat kematian sebelum bayi mencapai usia 8 minggu. Di samping itu, sebuah pengamatan jangka panjang terhadap populasi bayi dengan infeksi CMV kongenital yang hidup setelah pemberian ganciclovir intravena maupun valganciclovir oral menemukan kejadian sensorineural hearing loss (SNHL), cerebral palsy, dan gangguan fungsi kognitif pada 71% kasus. Kami melaporkan seorang bayi laki-laki dengan riwayat kelahiran cukup bulan, berat badan lahir cukup, hasil positif pada pemeriksaan imunoglobulin G (IgG) anti-CMV terhadap serum ibu, dan pemberian air susu ibu (ASI) eksklusif yang terdiagnosis infeksi CMV kongenital dengan SNHL sedang pada usia 2 bulan. Bayi mendapat pengobatan dengan ganciclovir intravena selama 14 hari dan valganciclovir oral selama 20 minggu serta tindakan-tindakan stimulatif. Pada akhir pengamatan selama 18 bulan, didapatkan perbaikan secara serologis dan tidak ditemukan adanya gangguan tumbuh-kembang.
Cytomegalovirus (CMV) infection constitutes one of the health problems, which are frequently found in a number of developing countries, including Indonesia. A previous study disclosed that in the absence of adequate intravenous ganciclovir administration, congenital CMV infection, which affects an infant, may result in death prior to the age of 8 weeks. In addition, a long-term observation of the population of infants with congenital CMV infection, who survived following the administration of intravenous ganciclovir and oral valganciclovir, revealed the presence of sensorineural hearing loss (SNHL), cerebral palsy, and cognitive impairment in 71% of cases. We reported a male infant with the history of term gestational age, normal birth weight, positive maternal anti-CMV immunoglobulin G (IgG), and exclusive breastfeeding, who had been diagnosed as congenital CMV infection with the mild SNHL at the age of 2 months. This infant received intravenous ganciclovir for 14 days, oral valganciclovir for 20 weeks, and stimulative exercises. At the end of the 18-month observation, the improvement was serologically obtained and there was no developmental delay.
